From Narcotics to No-Narcotics: Transforming Foot and Ankle Surgery in the Tri-State Area and the World

In recent years, the landscape of foot and ankle surgery has undergone a quiet revolution. As someone who’s been practicing in the New York-New Jersey region and beyond for years, I’ve had a front-row seat to this shift. Not so long ago, it was standard practice to rely heavily on narcotics for post-surgical pain management. Today, we’ve pivoted to a nearly pain-free, narcotic-free approach that benefits patients immensely.

The Old Days: A Heavy Reliance on Narcotics

When I first started my training, the use of narcotics was the norm. We would often prescribe them quite liberally, advising patients to "stay ahead of the pain" by taking medication regularly. We didn’t fully grasp then the addictive nature of these medications or the long-term consequences of their use. It was simply the way things were done.

A Turning Point: Exploring Alternatives

Fast forward about a decade into my own practice, and I began looking for ways to reduce patients’ reliance on narcotics. One of the early innovations was a local anesthetic injection given around the surgical site. This provided three to five days of pain relief, allowing patients to get through the worst of the post-op discomfort without reaching immediately for narcotics. It was a significant step forward, but it didn’t eliminate the need entirely.

The Multimodal Revolution

The real breakthrough came with the concept of multimodal pain management. Instead of relying on a single type of medication, we combined several non-narcotic medications at lower doses. Patients would receive a regimen of acetaminophen, anti-inflammatories, and other nerve-modulating medications both before and after surgery. This approach meant that when the local anesthetic wore off, patients already had a layered defense against pain. Over time, this multimodal method has allowed us to drastically cut down on narcotic use and, in many cases, eliminate it.

A New Era: Minimizing Risk, Maximizing Comfort

Today, our practice has effectively become a low-narcotic or even no-narcotic environment, by utilizing some of the novel medications to help patients manage their pain when the above strategies don't work. We’ve been following this approach for months with outstanding results. Patients recover comfortably, and we avoid the well-known risks of narcotic medications, from nausea and constipation to the much more serious issue of addiction.

Of course, no medication is entirely without side effects. But by using a balanced, multimodal approach, we’ve found a way to keep those risks minimal and keep our patients safer.
